Providence Portland Medical Center HPB Fellowship Program Curriculum The HPB Fellowship at Providence Portland Medical Center follows the AHPBA recommended fellowship curriculum. The fellows are provided with a copy of the Blumgart‘s Textbook of Surgery of Liver, Biliary Tract & Pancreas to guide their studying. It is a 2-year fellowship, with the first year being predominantly clinical with emphasis on inpatient management of the HPB service and obtaining required case numbers for completion of the fellowship. The second year is more flexible, allowing for specialization and focus on advanced minimally invasive techniques, additional adjunctive surgical experience (foregut, colorectal, general surgery, surgical oncology), basic science and clinical research opportunities, and multi-disciplinary rotations (medical oncology, advanced endoscopy, interventional radiology, and transplant). Schedule The fellows work with three dedicated hepatobiliary and surgical oncology fellowship trained faculty. There is one dedicated clinic day per week (Wednesday) and three operative days (Monday, Tuesday, and Friday). The remaining day (Thursday) is administrative, with HPB specific didactic and case review conferences, organized by the fellows with multi-disciplinary faculty involvement and teaching. Conferences There is a weekly surgical department M&M, and in January 2022 we will be implementing a separate monthly HPB M&M. There is a mandatory weekly 2-hour HPB focused multi-disciplinary tumor board at which the fellows present the surgical cases. Additionally, there is a weekly one-hour biliopancreatic conference with the advanced endoscopy group to discuss complex pancreatic and biliary cases. Also, there is a weekly GI tumor board attended by the HPB faculty focused on complex gastrointestinal cancers and colorectal cancer that the fellows are welcome to attend as well. The fellows attend the monthly AHPBA grand rounds. Journal Clubs/Research Meetings/National Conferences The fellows organize and participate in a bimonthly (every other month) journal club shared with the OHSU HPB Fellowship. Additionally, we have a bimonthly (every other month) research meeting, with weekly interim updates during case conference. Fellows are encouraged to submit their abstracts to national meetings. Attendance for at least one national meeting annually, as well as attendance at the annual ultrasound course is paid for and supported by the fellowship program. Clinical Exposure Our clinical practice is a high volume surgical oncology and hepatobiliary practice with incorporation of both benign and malignant pathology, including but not limited to pancreatic cancer, pancreatic cysts, acute and chronic pancreatitis and associated pancreatic duct/biliary strictures and pseudocyst management, neuroendocrine tumors, complex gallstone disease, cholangiocarcinoma, hepatocellular carcinoma, benign liver masses and cysts, metastatic colon cancer, metastatic neuroendocrine tumor, gastric cancer, retroperitoneal and extremity sarcoma, locally advanced adrenal cell carcinoma and renal cell carcinoma, duodenal cancer, GIST, and small bowel neuroendocrine tumors. The fellows can participate in a multi-disciplinary liver cancer clinic, run by hepatobiliary surgery and interventional radiology. This clinic focuses on patients with cirrhosis and hepatocellular carcinoma who are candidates for either surgery or catheter-based liver-directed therapy.
