Bile duct injury and repair (Draft) [Case 28]

Introduction

This patient is a 70-year old male generally in good condition. He underwent an elective laparosopic cholecystectomy after a bout of biliary pancreatitis. During the procedure, the surgeon encountered tight adhesions between the gallbladder and the color with abscesses in contact with the common bile duct. With difficulty, the lateral side of the common bile duct was injured and repaired with Vicryl suture.

Post-operatively, the patient had bile leak of 500ml/day in the drainage.

On Post-Operative Day 7, an MRI was ordered and showed that the confluence between the common bile duct and main hepatic duct was not visible.


With persistent bile leak, the patient was referred to an HPB centre on Post-Operative Day 11.
